Team Coordination

Definition

Team coordination is the structural mechanism through which a group of specialists acts as one. In The Checklist Manifesto it is the active ingredient implicated, by the WHO trial data, in the 36 percent drop in major surgical complications and the 47 percent drop in deaths — not the individual safety-step gains, which could not account for the size of the effect, but the broader communication improvement the checklist forced.

Team coordination, in Gawande's account, is built by four structural moves: introductions by name and role, brief team briefing on the case-specific risks, explicit dispersal of the authority to halt, and forced cross-discipline communication at defined pause-points. These look like social niceties; the evidence is that they are clinical interventions.

How it works

The four structural moves operate together; removing any one degrades the others.

  1. Name-and-role introductions convert anonymous specialists into named teammates and prime the activation phenomenon — the increased willingness to speak up later.

  2. Case- or shift-specific briefings integrate the specialists' knowledge before commitment. The surgeon names expected duration and blood-loss risk; anaesthesia names plan and concerns; nursing names equipment and patient concerns. The brief is short (under a minute) and forced (it happens at a defined pause-point).

  3. Dispersed authority ensures the pause can be called by someone other than the most senior person. Aviation's "pilot not flying calls the checklist" and the WHO list's "circulating nurse opens the checklist" both implement this.

  4. Forced cross-discipline communication — the construction industry's submittal schedule, the surgical pre-incision briefing — ensures the integration happens before commitment rather than during recovery.

The deeper claim is that team coordination is engineerable. The Hudson landing was not luck; it was the product of structural moves Sullenberger and Skiles had each run many times before. Tenerife was not bad luck either; it was the absence of the same structural moves.
